IMMUNIZATION REQUIREMENTS
All new students are required to enter immunization dates and submit a verified copy of their immunization record, and complete the online Tuberculosis Risk Assessment Questionnaire in My Health-e web portal. COVID-19 vaccine is not required for students. Students should upload their verified documents to the MyHealth-e web portal. Failure to comply with medical clearance requirements by established deadlines will result in a late fee and other administrative penalties detrimental to students’ continued enrollment at Clemson University.

Deadline to submit immunization requirements: July 1, 2022.
*Exception: Students enrolled in only online courses do not need to comply with medical clearance requirements.
HEALTH HISTORY
Our health care providers are looking forward to caring for your student during their time at Clemson University. We believe that having some baseline health information available will facilitate the best possible care. We encourage students to complete the Health History in MyHealth-e while they are entering their immunization information.
